M/THS Association directory

The following information is needed:

1. Present first and last names.

2. First and /or last names used at MHS/THS if different from present name (s).

3. Class or other connection with MHS/THS (faculty/administration staff, parent with special connection, or amigo/amiga). (for high school students, "class" is the year of graduation from high school, even if they didn't graduate from MHS/THS. Our definition of "MHS/THS" includes the 7th and 8th grades in years in which those grades were included in the yearbook.)

4. Complete mailing address.

The following information is optional - but please provide it all, if you have it ( you can ask that it not be included in the Directory.)

1. Home and work phone numbers.

2. Name and address of a parent or other person whose mailing address is more "permanent." (Used mainly to help find someone who becomes "lost"; this address won't be in the Directory, unless it's the only valid address we have.)

3. E-mail address
